MAYFIELD, EVERETT DALE born November 6, 1945 in Tahlequah, Oklahoma has passed away November 23, 2012 in Dallas, Texas. Everett is survived by his loving wife, Nancy; son, Christopher Mayfield and wife, Katie; daughter, Emily Mayfield; and granddaughter, Kayla. Visitation will be Tuesday, November 27, 2012 from 6:00 PM to 8:00 PM at Restland Funeral Home. Service will be held Wednesday, November 28, 2012 at 11:00 AM at Restland Memorial Chapel. Graveside service will follow at 1:30 PM at DFW National Cemetery.
