BALLEW, MARK SHELTON – Born October 19, 1953 in Memphis, TX and passed away June 30, 2012 in Addison, TX of prostate cancer. He was preceded in death by his father, Jack H. Ballew and nephew, Travis Ballew. Mark is survived by his mother, Carolyn Ballew; wife, Debra; son, Chad Ballew and wife, Jenny; daughter, Tracy Ballew Tobin and husband, Matt; grandchildren, Jude and Talula Tobin; brother, Mike Ballew and wife, Margo; sisters, Peggy Moore and husband, Joe, Sandra Werthmann and husband, Joe; niece, Raymi Moore and nephews, Jack Moore and Cole Werthmann. Service will be held at 10:00AM, Saturday, July 7, 2012 at Restland Memorial Chapel. A special thanks to everyone at Texas Oncology, Baylor Plano.
